Summary

Untreated sleep-disordered breathing (SDB) in children is linked to behavioral issues. Functional near-infrared spectroscopy (fNIRS) shows altered frontal lobe blood flow correlates with these behaviors, suggesting a potential biomarker.

Background:

  • The underlying neurological mechanisms linking SDB and behavioral issues remain unclear.

Purpose of the Study:

  • To investigate the relationship between frontal lobe cerebral hemodynamics and problem behaviors in children with SDB.
  • To assess the utility of functional near-infrared spectroscopy (fNIRS) as a biomarker for SDB-related adverse outcomes.

Main Methods:

  • A cross-sectional study enrolled 54 children (aged 5-16) with SDB.
  • Functional near-infrared spectroscopy (fNIRS) measured frontal lobe cerebral hemodynamics during polysomnography.
  • Parent-reported problem behaviors were assessed using the Behavioral Response Inventory of Executive Function Second Edition (BRIEF-2).

Main Results:

  • A significant inverse correlation was found between frontal lobe perfusion instability (measured by fNIRS) and BRIEF-2 clinical scales (r=0.24-0.49, p<0.001).
  • No significant correlation was observed between SDB severity (apnea-hypopnea index) and BRIEF-2 scales.

Conclusions:

  • Functional near-infrared spectroscopy (fNIRS) shows preliminary promise as a child-friendly biomarker for assessing adverse outcomes in children with SDB.
  • Altered frontal lobe hemodynamics may underlie the behavioral problems associated with SDB.
