Summary

This study shows that the Indigo Aspiration System effectively removed cerebral venous sinus thrombosis in a patient with a hemorrhagic infarct. Mechanical thrombectomy combined with anticoagulation led to successful recanalization and a good outcome.

Background:

  • Cerebral venous sinus thrombosis (CVST) can lead to severe neurological deficits and hemorrhagic infarcts.
  • Standard anticoagulation may be insufficient for high-risk CVST cases.
  • Minimally invasive endovascular techniques are being explored for CVST treatment.

Observation:

  • A 30-year-old female presented with seizures and focal neurological deficits.
  • Imaging revealed extensive CVST with a left posterior temporal lobe hemorrhagic infarct.
  • The patient was considered high-risk for poor response to anticoagulation alone.

Findings:

  • Successful endovascular thrombectomy was performed using Penumbra's Indigo Aspiration System (7F CAT7).
  • The procedure combined mechanical thrombectomy with heparin infusion.
  • Radiographic recanalization of the affected sinuses was achieved with near-complete thrombus removal.

Implications:

  • The Indigo Aspiration System demonstrated safety and efficacy in the cerebral venous system.
  • This minimally invasive approach offers a viable treatment option for complex CVST.
  • Successful recanalization correlated with an excellent functional outcome at follow-up.
