    Background:

    • Porta-caval shunts are used to treat portal hypertension.
    • Hepatic encephalopathy (HE) is a common complication following such procedures.
    • Assessing HE can be challenging, with varying sensitivity across diagnostic methods.

    Purpose of the Study:

    • To identify the most sensitive index for detecting hepatic encephalopathy in patients post-porta-caval shunt.
    • To compare the effectiveness of relative assessments with standard clinical and psychometric tests for HE.

    Main Methods:

    • The study involved 29 patients who underwent a porta-caval shunt.
    • Close relatives were interviewed for pre- and post-operative assessments of patient's mental state.
    • Standard clinical evaluations, trail tests, and the Eysenck personality questionnaire were administered.

    Main Results:

    • Relatives identified mental slowing in 19/20 patients, with behavioral changes (aggression or placidity) noted in many.
    • Clinical assessment detected encephalopathy in only 9 patients, while psychometric tests showed abnormalities in 14 (trail test) and 8 (five-pointed star).
    • Eighteen patients had elevated 'social naïvety' scores, and 8/11 aggressive patients showed abnormal scores, suggesting a link between personality changes and HE.

    Conclusions:

    • Close relative assessment appears to be a highly sensitive, non-invasive tool for detecting early hepatic encephalopathy post-porta-caval shunt.
    • Standard clinical and psychometric tests may underestimate the prevalence and impact of HE in these patients.
    • The findings highlight the importance of incorporating caregiver perspectives in the comprehensive evaluation of post-surgical hepatic encephalopathy.
