    Background:

    • Small vessel disease (SVD) is linked to diabetes, amyloidosis, and connective tissue diseases.
    • Its role in myocardial disease of unknown origin requires further investigation.

    Purpose of the Study:

    • To determine the incidence and morphology of SVD in patients with myocardial disease and normal epicardial coronary arteries.
    • To correlate clinical presentation and pathological changes of SVD.

    Main Methods:

    • Endomyocardial biopsy specimens from 110 consecutive patients were analyzed.
    • Light and electron microscopy were used to examine small vessel pathology.

    Main Results:

    • Small vessel abnormalities were found in 16 patients (14.6%).
    • Of these, 5 had hypertension and 11 had idiopathic SVD.
    • Morphologically, SVD showed marked arterial wall thickening from subendothelial deposits.

    Conclusions:

    • Small vessel disease is a significant finding in unexplained myocardial disease.
    • Subendothelial deposits characterize the observed SVD morphology.
    • Further research is needed to understand the clinical implications of SVD in this population.
