Pulmonary Hypertension: Classification and Pathogenesis01:30

Pulmonary Hypertension: Classification and Pathogenesis

241
Pulmonary hypertension (PH) is a severe health condition in which the mean pulmonary arterial pressure increases to 25 mmHg or more, even when the body is at rest. This high pressure in the blood vessels that transport blood from the heart to the lungs can cause various symptoms, including shortness of breath, can lead to right heart failure, and significantly affect the overall quality of life.
There are various classifications for PH, each relating to different underlying causes and also...

Radiological Investigation III: Pulmonary Angiogram and PET Scan

120
Radiological investigations are paramount in the diagnosis and management of various pulmonary diseases. Two essential investigations are the Pulmonary Angiogram and the Positron Emission Tomography (PET) Scan.
Pulmonary Angiogram
A Pulmonary Angiogram is an invasive procedure involving injecting a contrast medium through a catheter threaded into the pulmonary artery or the right side of the heart to visualize the pulmonary vasculature. Computed Tomography (CT) scans have mainly replaced this...

Summary
This summary is machine-generated.

Primary pulmonary artery sarcoma (PAS) is a rare tumor often misdiagnosed as pulmonary embolism (PE). Early recognition of PAS, distinct from PE, is crucial for timely surgical intervention and improved patient outcomes.

Area of Science:

  • Cardiovascular Pathology
  • Thoracic Oncology
  • Diagnostic Imaging

Background:

  • Primary pulmonary artery sarcoma (PAS) is a rare malignancy.
  • PAS frequently mimics symptoms and imaging findings of pulmonary embolism (PE).
  • Chondrosarcoma within the pulmonary artery is an exceptionally rare subtype of PAS.

Observation:

  • A 67-year-old woman presented with symptoms suggestive of PE, including cough, chest pain, and shortness of breath.
  • Initial diagnostic imaging (CTPA) revealed filling defects in the pulmonary arteries, leading to a misdiagnosis of PE.
  • The patient underwent interventional treatment for PE with suboptimal response.

Findings:

  • Histopathological examination confirmed primary pulmonary artery chondrosarcoma after surgical resection.
  • The patient experienced tumor recurrence within 10 months, followed by lung metastasis.
  • Despite adjuvant chemotherapy, the patient succumbed to heart and respiratory failure two years post-surgery.

Implications:

  • Clinical and radiological features of PAS can be mistaken for PE, necessitating a high index of suspicion.
  • Delayed diagnosis due to misinterpretation as PE can lead to inappropriate treatments and poorer prognosis.
  • Alertness to PAS in cases of suspected PE with poor response to anticoagulation/thrombolysis is vital for early diagnosis and effective management.
