Abstract

Insights

Donafenib significantly improved progression-free survival (PFS) in Chinese patients with radioactive iodine-refractory differentiated thyroid cancer (RAIR-DTC). This oral tyrosine kinase inhibitor (TKI) offers a new treatment option for this patient population.

Area of Science:

  • Oncology
  • Pharmacology
  • Clinical Trials

Background:

  • No established treatments existed for progressive radioactive iodine-refractory differentiated thyroid cancer (RAIR-DTC) in Chinese patients.
  • Phase II studies indicated donafenib, an oral tyrosine kinase inhibitor (TKI), showed promise in efficacy and tolerability.

Purpose of the Study:

  • To evaluate the antitumor activity and safety of donafenib in Chinese patients with progressive RAIR-DTC.
  • To compare donafenib's efficacy against a placebo in this specific patient group.

Main Methods:

  • A multicenter, double-blind, placebo-controlled, phase III study randomized 191 patients with progressive RAIR-DTC.
  • Patients received either donafenib (300 mg twice daily) or a matched placebo in a 2:1 ratio.
  • Primary endpoint was progression-free survival (PFS); secondary endpoints included objective response rate (ORR) and disease control rate (DCR).

Main Results:

  • Donafenib significantly prolonged median PFS compared to placebo (12.9 vs. 6.4 months; HR=0.39, P<0.0001).
  • Improved objective response rate (ORR) (23.3% vs. 1.7%; P=0.0002) and disease control rate (DCR) (93.3% vs. 79.3%; P=0.0044) were observed with donafenib.
  • Common grade ≥3 adverse events included hypertension and hand-foot syndrome; dose modifications were frequent but discontinuation was low.

Conclusions:

  • Donafenib demonstrated significant clinical benefit, improving PFS, ORR, and DCR in Chinese patients with RAIR-DTC.
  • The drug was generally well-tolerated, suggesting it as a viable new treatment option for RAIR-DTC.
