A rare internal hernia (IHAC) complication after rectal cancer surgery caused small bowel obstruction. Laparoscopic surgery successfully repaired the hernia, offering a less invasive option for future cases.
Area of Science:
- Gastroenterology
- Surgical Oncology
- Abdominal Surgery
Background:
- Rectal cancer treatment, including abdominoperineal resection, can lead to rare complications.
- Internal hernias after colostomy are uncommon but can cause significant morbidity.
Observation:
- A 63-year-old woman presented with recurrent small bowel obstruction two months post-laparoscopic abdominoperineal resection.
- Imaging revealed internal hernia with dilated small intestine through a defect between the sigmoid colon and abdominal wall.
Findings:
- Initial nonoperative management was unsuccessful due to recurrent obstruction.
- Laparoscopic surgery successfully reduced the herniated intestine and closed the defect with barbed suture.
Implications:
- Laparoscopic surgery is a feasible and potentially less invasive approach for managing internal hernias after colostomy.
- This technique may reduce the need for extensive adhesiolysis in re-operative cases.
