Retrosternal goiter and chylothorax: case report.

L Fernandez-Cruz, J Serra-Batlles, C Picado

Respiration; International Review of Thoracic Diseases

    Summary

A rare case of chylothorax, a buildup of fluid in the chest, was linked to a retrosternal goiter. This previously unreported association suggests the goiter likely compressed key thoracic vessels, causing the condition.

    Area of Science:

    • Cardiology
    • Thoracic Surgery
    • Endocrinology

    Background:

    • Retrosternal goiters, enlarged thyroid glands extending behind the sternum, can cause compressive symptoms.
    • Chylothorax is the accumulation of lymphatic fluid in the pleural space, often due to thoracic duct disruption.

    Observation:

    • A patient presented with chylothorax.
    • The patient also had a retrosternal goiter.

    Findings:

    • This is the first reported instance of chylothorax associated with a retrosternal goiter.
    • The chylothorax was likely caused by the goiter compressing the thoracic duct and brachiocephalic vessels.

    Implications:

    • This case highlights a novel complication of retrosternal goiters.
    • It emphasizes the importance of considering vascular and lymphatic compression in the etiology of chylothorax.
    • Surgical or medical management of the goiter may resolve the chylothorax.
