Background:

Emphysematous epididymo-orchitis is a rare, life-threatening infection characterized by gas within the epididymis and testes.
It often affects patients with underlying conditions such as diabetes mellitus.

Observation:

A 49-year-old male with diabetes and hypertension presented with acute right inguinoscrotal swelling and severe pain.
Scrotal ultrasound revealed a fluid-filled avascular mass.
Non-contrast CT scan demonstrated gas and fluid density in the right epididymis, perineum, and spermatic cord.

Findings:

The patient was diagnosed with emphysematous epididymo-orchitis.
After initial refusal, the patient consented to surgical intervention.
A right orchidectomy with spermatic cord removal was successfully performed.

Implications:

This case underscores the importance of prompt diagnosis and surgical intervention for emphysematous epididymo-orchitis.
Early recognition and management are crucial to prevent severe complications and mortality.
Surgical management, including orchidectomy, can be effective in treating this rare condition.
