    Summary

    Rapid atrial pacing effectively converts atrial flutter to normal sinus rhythm in most medically treated patients. This safe and convenient method is a viable alternative to transthoracic cardioversion.

    Background:

    • Atrial flutter is a common arrhythmia, often managed with antiarrhythmic drugs.
    • Transthoracic cardioversion carries risks and may not be suitable for all patients.

    Purpose of the Study:

    • To evaluate the efficacy and safety of rapid atrial pacing for elective conversion of atrial flutter.
    • To identify factors influencing the success of atrial pacing in patients with atrial flutter.

    Main Methods:

    • Fifty-seven episodes of atrial flutter in 46 medically treated patients were treated with rapid atrial pacing.
    • Patients often received concurrent digoxin and antiarrhythmic drug therapy.
    • Success rates were analyzed, considering patient demographics, cardiac history, and pacing techniques.

    Main Results:

    • Successful conversion to normal sinus rhythm was achieved in 89% of pacing trials.
    • Older patients with congestive heart failure were more likely to be refractory to pacing.
    • Sinus node suppression occurred in 17% of patients, managed easily with the pacing catheter.

    • Atrial pacing is an effective, safe, and convenient method for elective atrial flutter conversion in medically treated patients.
