Purpose of the Study:

  • To investigate the influence of age at diagnosis on breast cancer-specific survival (BCSS) and overall survival (OS).
  • To identify distinct clinical-pathological and treatment patterns across different age cohorts.

Main Methods:

  • Retrospective population-based study of 24,469 invasive breast cancer patients diagnosed between 2005-2014.
  • Analysis of clinical, pathological, and treatment variables across age groups: <35, 35-39, 40-49, 50-59, 60-69, 70-79, and ≥80.
  • Assessment of age impact on BCSS and OS, controlling for subtype, lymphovascular invasion (LVI), stage, and treatment.

Main Results:

  • Younger patients (<35, 35-39) often presented with higher-risk features (HER2+, triple-negative, advanced stage) and received more aggressive treatments.
  • Elderly patients (≥80) were more likely to have hormone-sensitive, HER2-negative, lower-stage disease and received less intensive treatment.
  • Both extremes of age at diagnosis were identified as independent risk factors for poorer prognosis.

Conclusions:

  • Age at diagnosis is a critical, independent prognostic factor in breast cancer.
  • Distinct clinical and treatment disparities exist at the extremes of age.
  • Findings can aid clinicians in refining outcome predictions and tailoring evidence-based treatment recommendations.
