Summary

Valve sparing root replacement (VSRR) using reimplantation offers excellent long-term results with low reintervention rates. This technique maximizes annular support for patients with annuloaortic ectasia.

Reimplantation VSRR is favored for optimal annular support in annuloaortic ectasia.
Surgical variations include graft selection, suture techniques, and annular stabilization strategies.

Discussion:

Our institution has refined a specific reimplantation VSRR technique over 18 years.
The current approach utilizes a larger straight graft, six inflow sutures, and annular plication/stabilization.
This standardized method aims for consistent and reproducible surgical outcomes.

Key Insights:

The refined reimplantation VSRR technique demonstrates low reintervention rates for both trileaflet and bicuspid valves.
Maximum annular support is achieved through this specific VSRR approach.
Long-term efficacy of VSRR is supported by favorable reoperation data.

Outlook:

Further long-term outcome analysis of this VSRR technique is warranted.
Potential for wider adoption of this refined reimplantation strategy.
