Aneurysm III: Interprofessional Care

12
Aneurysm management involves either conservative medical therapy or surgical intervention, depending on the size and symptoms of the aneurysm. Conservative management is generally reserved for smaller, asymptomatic aneurysms, while larger or symptomatic aneurysms often necessitate surgical repair.Conservative Medical TherapyFor small, asymptomatic aneurysms, particularly abdominal aortic aneurysms (AAA) less than 5.5 centimeters in diameter, conservative medical therapy is recommended. This...

Acute Coronary Syndrome IV: Interprofessional Care

13
IntroductionThe management of Acute Coronary Syndrome (ACS) aims to minimize myocardial damage, preserve myocardial function, and prevent complications.Initial ManagementInpatient management involves continuous cardiac monitoring, preferably in an ICU, focusing on blood pressure, serum sodium, potassium, and creatinine levels, and urine output. Ongoing pharmacologic management is crucial for stabilizing the patient.Supplemental Oxygen: Administer supplemental oxygen if oxygen saturation is...

Coronary Artery Disease V: Interprofessional Care

11
Interprofessional care for coronary artery disease includes pharmacological therapy and revascularization procedures.Pharmacological therapy for Coronary Artery Disease (CAD) aims to manage symptoms, prevent complications, and improve patient outcomes through various classes of medications:Antiplatelet Agents:Aspirin and Clopidogrel: These medications inhibit platelet aggregation, preventing blood clots, which is crucial for avoiding heart attacks and strokes. Doctors often prescribe these...

Peripheral Artery Disease III: Interprofessional Care

13
Peripheral Artery Disease (PAD) is characterized by narrowed arteries that diminish blood flow to the extremities. Effective management of PAD requires an interprofessional approach involving various healthcare professionals. The critical aspects of interprofessional care for PAD patients focus on risk factor modification, drug therapy, exercise therapy, nutrition therapy, critical limb ischemia care, and interventional radiology and surgical procedures. The primary treatment goal for PAD...

Electronic Medical Records (EMRs) primarily center around electronically documenting patients' health information within a single healthcare organization or practice. They contain essential clinical data related to a patient's medical history, diagnoses, medications, treatment plans, lab results, and other pertinent information relevant to the specific encounter or episode of care. EMRs are designed to streamline documentation and workflow processes within individual healthcare...

Acute Kidney Injury (AKI) requires a collaborative healthcare approach to restore renal function and prevent complications. Essential management strategies involve monitoring fluid and electrolyte balance, adjusting medications, initiating dialysis when necessary, and providing nutritional support.Fluid and Electrolyte ManagementFluid Monitoring: Regularly monitoring body weight, central venous pressure, and urine output helps detect fluid imbalances early. Patient intake and output are...

Summary

A new, unified medical summary form (VSM) was developed for dependent elderly care facilities (Ehpad) to address its previous scarcity. This standardized VSM has shown positive user feedback and is now being implemented in the Île-de-France region.

Area of Science:

  • Geriatric Medicine
  • Health Informatics
  • Healthcare Management

Context:

  • The medical summary form (VSM) was established in 2011 and refined in 2013.
  • VSMs are largely absent in French residential facilities for dependent elderly people (Ehpad).
  • Physicians frequently require VSMs, especially during emergencies for resident care.

Purpose:

  • To address the critical lack of standardized medical summary forms in Ehpad settings.
  • To create a unified VSM that meets the practical needs of healthcare professionals in elderly care.
  • To improve emergency medical care for residents in dependent care facilities.

Summary:

  • A working group convened in 2021 to develop a unique VSM for Ehpad, following the health crisis.
  • The newly created VSM underwent testing and received highly favorable user feedback.
  • This standardized VSM is currently being deployed across Ehpad facilities in the Île-de-France region.

Impact:

  • Enhances the quality of emergency medical care for elderly residents in Ehpad.
  • Improves information accessibility for physicians managing patient care in residential facilities.
  • Facilitates better healthcare coordination and patient safety within dependent elderly care settings.
