In Vitro Fertilization01:24

In Vitro Fertilization

337
In vitro fertilization (IVF) is a form of assisted reproductive technology where an egg is fertilized with sperm in a controlled laboratory environment before transferring the resulting embryo into the uterus. This process is designed to help individuals and couples experiencing difficulties conceiving.
The IVF process begins with ovarian stimulation, during which reproductive endocrinologists prescribe hormonal medications to stimulate the ovaries to produce multiple eggs instead of the single...

Summary

Recurrent implantation failure (RIF) is now defined by ESHRE as a secondary phenomenon of assisted reproductive technology (ART). New guidelines recommend investigations and treatments for RIF based on a cumulative implantation chance threshold of 60%.

Area of Science:

  • Reproductive Medicine
  • Assisted Reproductive Technology (ART)
  • Implantation Science

Background:

  • Recurrent implantation failure (RIF) presents a significant challenge in ART clinics.
  • Numerous investigations and interventions are employed without clear evidence of benefit.

Purpose of the Study:

  • To establish a clear definition for RIF in patients undergoing ART.
  • To provide recommendations for investigating RIF causes and management strategies.
  • To enhance pregnancy success rates in cases of RIF.

Main Methods:

  • Development of ESHRE good practice recommendations.
  • Literature review focusing on 'recurrent implantation failure'.
  • Expert consensus from a multidisciplinary working group.
  • Incorporation of survey data on clinical practices and peer review.

Main Results:

  • RIF is defined as a secondary phenomenon of ART, observed in IVF patients.
  • A cumulative predicted chance of implantation of 60% is recommended as a threshold for further investigation.
  • Nineteen recommendations for investigations and 13 for interventions were formulated, color-coded by evidence level.

Conclusions:

  • RIF identification should be based on individual implantation chance.
  • Investigations and treatments should be restricted to those with clear rationale and evidence of benefit.
  • Further research is crucial for advancing RIF clinical management.
