Background:

  • Acute macular neuroretinopathy (AMN) is a rare condition affecting the retina.

Observation:

  • A 28-year-old woman developed bilateral paracentral vision scotomata two days post-SARS-CoV-2 vaccination (Vaxzevria).
  • Clinical examination revealed petaloid lesions around the fovea.
  • Spectral domain optical coherence tomography showed a hyperreflective plaque and disruption of retinal layers.

Findings:

  • Diagnosis of acute macular neuroretinopathy (AMN) was confirmed.
  • The patient was otherwise healthy, suggesting a direct link to vaccination.
  • The findings highlight specific OCT characteristics of vaccine-associated AMN.

Implications:

  • Acute macular neuroretinopathy may be an extremely rare adverse event associated with SARS-CoV-2 vaccination.
  • This case underscores the importance of monitoring for rare ocular side effects post-vaccination.
  • Further research is needed to elucidate the mechanism and incidence of vaccine-related AMN.
