    Summary

    Epstein-Barr viral (EBV) stromal keratitis presents uniquely in patients, differing from other viral causes. This research suggests EBV is a more frequent cause of stromal keratitis than previously understood.

    Background:

    • Stromal keratitis is a significant cause of corneal opacity and vision impairment.
    • Epstein-Barr virus (EBV) is a common human herpesvirus, typically associated with infectious mononucleosis.
    • The ocular manifestations of EBV are diverse, but its role in stromal keratitis requires further elucidation.

    Observation:

    • Seven patients with clinical and serological evidence of Epstein-Barr viral (EBV) stromal keratitis were identified.
    • Distinct presentations included multifocal, pleomorphic stromal opacities and peripheral infiltrates resembling syphilitic keratitis.
    • One patient exhibited features of both described patterns.

    Findings:

    • EBV-associated stromal keratitis can manifest with unique granular or blotchy infiltrates.
    • The observed patterns appear distinct from those typically seen in adenoviral or herpes simplex viral keratitis.
    • The findings suggest EBV may be an underrecognized cause of stromal keratitis.

    Implications:

    • Increased awareness of EBV as a potential cause of stromal keratitis is warranted.
    • Diagnostic criteria may need refinement to include EBV in the differential diagnosis of stromal keratitis.
    • Further research into the pathogenesis and treatment of EBV stromal keratitis is recommended.
