Summary

Promoting evening laboratory draws did not increase hemoglobin levels in hospitalized patients. While evening draws showed higher hemoglobin, the intervention did not significantly alter overall patient hemoglobin measurements.

Background:

  • Hospital-acquired anemia is a frequent complication of hospitalization, potentially increasing transfusion needs and length of stay.
  • Recumbent (lying down) posture is known to falsely lower hemoglobin measurements.

Purpose of the Study:

  • To investigate if an initiative promoting evening laboratory blood draws could improve hemoglobin measurements in hospitalized patients.
  • To assess if increased upright posture time during the day and evening, due to later blood draws, impacts hemoglobin levels.

Main Methods:

  • A quasi-experimental study was conducted on two medical units from March 2020 to January 2021.
  • One unit implemented an evening routine laboratory draw initiative in August 2020.
  • Hemoglobin levels were analyzed using linear mixed-effects models and difference-in-difference analysis before and after the intervention.

Main Results:

  • In the pre-intervention phase, evening blood draws were associated with higher hemoglobin levels (0.28 g/dL higher).
  • The intervention significantly increased the proportion of evening blood draws from 10.3% to 47.9%.
  • However, the difference-in-difference analysis showed no significant association between the intervention and hemoglobin levels (-0.15 g/dL).

Conclusions:

  • Evening laboratory blood draws are independently associated with higher hemoglobin measurements.
  • Despite successful implementation of an evening draw initiative, it did not lead to a significant increase in overall patient hemoglobin levels.
