Overlapping stent-assisted coil embolization using low-profile visualized intraluminal support (LVIS) stents effectively treated large unruptured aneurysms. This technique offers improved flow diversion for complex cases.
Area of Science:
- Neurosurgery
- Interventional Neuroradiology
- Vascular Neurology
Background:
- Large unruptured intracranial aneurysms pose significant treatment challenges.
- Traditional stent-assisted coil embolization (SACE) may have limitations for complex aneurysm morphologies.
- Low-profile visualized intraluminal support (LVIS) stents offer advanced device options for endovascular repair.
Observation:
- Two patients with large unruptured aneurysms were treated with overlapping LVIS stent-assisted coil embolization (LSACE).
- Case 1 involved an internal carotid artery aneurysm presenting with abducens nerve palsy.
- Case 2 involved a partially thrombosed fusiform vertebral artery aneurysm.
Findings:
- Immediate digital subtraction angiography (DSA) showed residual dome filling in both cases.
- Follow-up DSA several months post-procedure demonstrated complete aneurysm occlusion.
- Overlapping LSACE achieved successful aneurysm obliteration with favorable long-term results.
Implications:
- Overlapping LSACE represents a potentially effective strategy for complex unruptured aneurysms.
- This technique may provide superior flow-diverting effects compared to standard SACE.
- LSACE expands endovascular treatment options for challenging intracranial aneurysms.
