    Congenital methaemoglobinaemia, caused by methaemoglobin reductase deficiency, leads to elevated oxidized hemoglobin. This report documents the first confirmed case in Southeast Asia, highlighting its rarity in non-Caucasian populations.

    • Methaemoglobin (metHb) is an oxidized form of hemoglobin, normally maintained at <1% in human blood.
    • Congenital methaemoglobinaemia results from impaired reduction of metHb due to enzyme deficiencies, primarily methaemoglobin reductase.
    • Elevated metHb levels (>10%) cause cyanosis and reduce oxygen-carrying capacity.
