Endocrine responses and examination anxiety.

J Herbert, G F Moore, C de la Riva

    Summary

Exam stress significantly impacts male medical students, increasing anxiety and elevating stress hormones like cortisol and prolactin. While personality traits influenced these endocrine changes, psychological coping strategies did not predict the hormonal response to examinations.

    Area of Science:

    • Endocrinology
    • Psychology
    • Medical Education

    Background:

    • Examinations are significant stressors for students.
    • Understanding the physiological and psychological responses to academic stress is crucial.

    Purpose of the Study:

    • To investigate the endocrine and psychological responses of male medical students before a major examination.
    • To explore the relationship between psychological factors and hormonal changes during academic stress.

    Main Methods:

    • Studied 38 male medical students 4 weeks and 1-2 hours before an examination.
    • Measured psychological variables (anxiety, coping) and endocrine markers (serum cortisol, prolactin, testosterone, LH; urinary noradrenaline, adrenaline).

    Main Results:

    • Anxiety and emotionality increased before the exam.
    • Serum cortisol and prolactin increased, as did urinary noradrenaline and adrenaline.
    • Increased cortisol correlated with increased prolactin; urinary catecholamines also correlated, but these two pairs of responses were not linked.

    Conclusions:

    • Academic stress elicits significant endocrine changes in medical students.
    • Certain personality traits, but not state anxiety or coping strategies, predicted hormonal responses.
    • Psychological measures did not predict the magnitude of the hormonal response to examinations in this student cohort.
