Summary

Most apparently healthy adults have silent liver conditions like steatosis and fibrosis. Vibration-controlled transient elastography and controlled attenuation parameter (CAP) show promise for noninvasive screening of these liver diseases in the general population.

Purpose of the Study:

  • To screen asymptomatic, apparently healthy subjects for significant hepatic fibrosis and steatosis.
  • To evaluate the utility of Vibration-controlled transient elastography and Controlled Attenuation Parameter (CAP) for this screening purpose.

Main Methods:

  • Prospective study including 433 asymptomatic, healthy adults.
  • All participants underwent Fibroscan/CAP examination.
  • Subjects with liver stiffness measurement > 6 kPa or CAP > 248 dB/m were further evaluated for chronic liver disease.

Main Results:

  • 72% of subjects exhibited significant steatosis and/or fibrosis.
  • Specific subgroups identified: normal (n=119), steatosis only (n=133), steatosis and fibrosis (n=95), fibrosis only (n=86).
  • Subjects with both steatosis and fibrosis had elevated transaminases, triglycerides, and total cholesterol.

Conclusions:

  • A high prevalence of significant steatosis and fibrosis was found in apparently healthy subjects.
  • Liver stiffness measurement and CAP are promising noninvasive tools for screening silent liver diseases.
  • These methods may serve as effective first-line procedures for population-based liver health assessment.
