Predictive and diagnostic measures for kernicterus spectrum disorder: a prospective cohort study

Neonatal brain MRIs are unreliable for predicting kernicterus spectrum disorder (KSD). The modified Bilirubin-Induced Neurologic Dysfunction (BIND-M) scale effectively screens for KSD, while the Barry-Albright Dystonia (BAD) scale or Hyperbilirubinemia Imaging Rating Tool (HIRT) can confirm diagnosis.

- Neonatal MRI (HIRT score 2) had 67% sensitivity and 75% specificity, improving to 100% at 12 months. BAD ≥2 was 100% specific for KSD from 3-12 months.
Conclusions:
- Neonatal brain MRI is not a reliable predictor of KSD.
- The modified BIND-M scale is a valuable screening tool for KSD risk.
- The BAD scale or HIRT score at 3 or 12 months can confirm KSD, enabling early diagnosis and intervention.
