Hospital integration does not significantly improve Medicare Merit-Based Incentive Payment System (MIPS) quality performance. This study found no difference in MIPS quality scores between integrated and independent physician practices.
Area of Science:
- Health Services Research
- Healthcare Management
- Value-Based Care
Background:
- Hospital-physician integration is often promoted for enhancing clinical quality through shared resources.
- Value-based programs, like the Medicare Merit-Based Incentive Payment System (MIPS), aim to link financial rewards to clinical quality.
- Integrated practices may potentially benefit from these value-based programs.
Purpose of the Study:
- To evaluate the association between hospital-physician integration and performance in the MIPS program.
- To compare MIPS performance metrics between hospital-integrated and independent physician practices.
Main Methods:
- Cross-sectional study utilizing 2020 Quality Payment Program data.
- Included physician practices with valid MIPS composite scores.
- Defined hospital integration based on billing practices (≥75% services via hospital outpatient codes or tax identifier).
Main Results:
- No significant difference in the overall MIPS quality category score was found between hospital-integrated and independent practices.
- This held true for both unadjusted and adjusted analyses (controlling for practice size, geography, specialty, and case mix).
- Hospital-integrated practices showed higher scores on some, but not all, specific quality measures.
Conclusions:
- Hospital integration does not appear to offer a substantial advantage for MIPS quality performance.
- The study suggests that integration alone may not be sufficient to drive superior quality outcomes within this specific value-based payment model.
