Summary

Switching from rifampin to rifabutin successfully enabled therapeutic anticoagulation in a patient on warfarin for pulmonary embolism. Rifabutin may be a safer alternative when rifamycin use is necessary alongside warfarin.

Background:

  • Managing concomitant warfarin and rifampin therapy can be challenging due to drug-drug interactions.

Observation:

  • A patient with PE on warfarin and active pulmonary tuberculosis (PTB) treated with rifampin failed to achieve therapeutic anticoagulation.
  • Warfarin dosage was progressively increased without reaching the target International Normalized Ratio (INR).

Findings:

  • Switching rifampin to rifabutin, an alternative rifamycin, resulted in achieving the target INR within six days.
  • A lower warfarin total weekly dose was required after the switch to rifabutin.

Implications:

  • Rifabutin exhibits a less significant drug-drug interaction with warfarin compared to rifampin.
  • Rifabutin can be considered as a safer alternative for patients requiring a rifamycin concurrently with warfarin therapy.
  • This case highlights a successful clinical intervention for managing complex drug interactions in anticoagulated patients.
