    Summary

    Nitrous oxide administration during cervical cerclage procedures under general anesthesia did not correlate with increased fetal abnormalities or adverse birth outcomes. The study found no evidence to support avoiding nitrous oxide in early pregnancy.

    Background:

    • Cervical cerclage is a procedure to prevent preterm birth.
    • Concerns exist regarding the safety of anesthetic agents, including nitrous oxide, during pregnancy.
    • Previous studies have suggested potential risks of nitrous oxide in early gestation.

    Purpose of the Study:

    • To evaluate the safety of nitrous oxide administration during surgical procedures in pregnant women.
    • To assess the association between nitrous oxide use and fetal abnormalities or adverse pregnancy outcomes.
    • To test the proposition that nitrous oxide should be avoided in the first or second trimester of pregnancy.

    Main Methods:

    • Analysis of 375 cases (383 fetuses) undergoing cervical cerclage under general anesthesia with nitrous oxide.
    • Comparison of outcomes with 58 other operations (59 fetuses) under general anesthesia including nitrous oxide.
    • Comparison of outcomes in cervical cerclage cases under general anesthesia with nitrous oxide versus regional analgesia (115 fetuses).

    Main Results:

    • No instances of fetal abnormality were clearly attributable to nitrous oxide administration.
    • The incidence of inevitable abortion and low birth weight was identical between cervical cerclage under general anesthesia with nitrous oxide and regional analgesia.
    • This study provides interim findings for a two-part investigation.

    • The proposition that nitrous oxide should not be administered to women in the first or second trimester is deemed unacceptable based on current findings.
    • Nitrous oxide appears safe for use during cervical cerclage and other surgical procedures in early pregnancy.
    • Further research is warranted to fully elucidate the safety profile of nitrous oxide in obstetric anesthesia.
