Background:

  • Recurrent cystitis management guidelines suggest using local antibiograms or prior urine cultures for empirical antibiotic prescribing.
  • Limited data exist on the precise predictive value of prior urine cultures in guiding treatment for recurrent cystitis.

Purpose of the Study:

  • To construct a urinary antibiogram for patients with recurrent, uncomplicated cystitis.
  • To evaluate the test metrics (sensitivity, specificity, predictive values) of prior gram-negative organism results in predicting subsequent resistance or susceptibility.

Main Methods:

  • Retrospective database study of adult patients with recurrent, uncomplicated cystitis (2/6 months or 3/12 months).
  • Data collected from urology or primary care clinics between November 2016 and December 2018.
  • Exclusion criteria included pregnant patients, complicated cystitis, or pyelonephritis. Test metrics calculated using standard formulas for sequential, paired cultures.

Main Results:

  • Analysis included 597 visits from 232 patients; 310 visits had urine cultures, with 165 yielding gram-negative uropathogens.
  • Escherichia coli isolates showed resistance rates of 38.0% to trimethoprim-sulfamethoxazole, 27.9% to ciprofloxacin, and 5.5% to nitrofurantoin.
  • Prior cultures (within 2 years) demonstrated good predictive value for susceptibility to nitrofurantoin (0.85) and trimethoprim-sulfamethoxazole (0.78), and excellent predictive values (≥0.90) for several other antibiotics.

Conclusions:

  • Significant antibiotic resistance was observed in E. coli isolates from patients with recurrent, uncomplicated cystitis.
  • Utilizing prior urine culture results can improve the selection of effective empirical antibiotic agents for recurrent urinary tract infections.
