Thyroid autoimmunity, particularly anti-thyroid peroxidase (TPO) antibodies, may predict late postpartum depression (PPD). A postpartum cortisol drop can trigger thyroid autoimmunity in TPO-positive women, leading to hypothyroidism and depressive symptoms.
