    Background:

    • Exercise stress testing is a common diagnostic tool.
    • Assessing the safety and arrhythmogenic potential of exercise testing is crucial.

    Purpose of the Study:

    • To evaluate the incidence of ventricular arrhythmias during and after treadmill exercise tests.
    • To determine the safety of exercise stress testing in a large patient cohort.

    Main Methods:

    • Retrospective review of 2,400 treadmill exercise tests performed between 1973 and 1982.
    • Identification of ventricular tachycardia (VT) and ventricular fibrillation (VF) episodes within 8 minutes of exercise cessation.

    Main Results:

    • VT occurred in 4.0% and VF in 0.7% of patients with coronary artery disease (CAD).
    • VT occurred in 0.07% of patients without known CAD; no VF was observed.
    • Most arrhythmias resolved spontaneously, with only 5 patients requiring cardioversion.

    • Exercise stress testing is a safe procedure with a low incidence of serious ventricular arrhythmias.
    • Patients with known coronary artery disease represent the primary group at risk for exercise-induced VT/VF.
