Background:

Hallucinogen-persisting perception disorder (HPPD) is a neurological condition characterized by persistent perceptual disturbances following hallucinogen use.
Lysergic acid diethylamide (LSD) and similar substances are frequently implicated in the development of HPPD.

Observation:

A 28-year-old male with a history of polysubstance use, including LSD and cannabis, developed classic HPPD symptoms seven months after discontinuing LSD.
The patient's symptoms persisted despite treatment with antipsychotics, suggesting a cause beyond underlying mental illness.

Findings:

Diagnosis of HPPD was confirmed by excluding other potential medical and neurological causes through laboratory and imaging tests.
Lamotrigine, typically used for perceptual disturbances, demonstrated significant improvement in the patient's HPPD symptoms, challenging conventional treatment approaches.

Implications:

This case suggests lamotrigine may be an effective therapeutic option for managing HPPD, particularly when first-line treatments like alpha-2 adrenergic drugs or benzodiazepines are ineffective.
Further research into lamotrigine's efficacy and mechanism of action in HPPD is warranted to establish its role in clinical practice.
