    Transitional cell carcinoma can metastasize to the brain as a single lesion. Surgical resection and radiotherapy offer limited survival benefits for these rare brain metastases.

    • Transitional cell carcinoma (urothelial carcinoma) is a malignancy originating in the urinary tract.
    • Brain metastases from transitional cell carcinoma are rare, with limited data on management and outcomes.

    Observation:

    • Two cases of transitional cell carcinoma presenting with a single brain metastasis are described.
    • Both patients underwent surgical resection of the brain lesion followed by whole brain radiotherapy.

    Findings:

    • One patient developed leptomeningeal carcinomatosis and died within 4 months.
    • The second patient survived for 1 year with no evidence of intracranial disease, despite metastases in other organs.
