Background:

  • Cost-related medication nonadherence (CRN) is a critical patient-centered outcome.
  • Longitudinal studies tracking CRN are scarce, limiting understanding of its temporal patterns.

Purpose of the Study:

  • To develop an integrated dataset for longitudinal CRN research.
  • To characterize CRN patterns and identify associated factors in high-risk Medicare beneficiaries.

Main Methods:

  • A cohort of 2000 high-risk Medicare beneficiaries was surveyed quarterly for 8 quarters (2013-2018) on CRN.
  • The dataset was linked to Medicare files, and a CRN metric (persistent, intermittent, transient) was developed.
  • Ordered logit and logit models assessed factors influencing CRN and persistent CRN.

Main Results:

  • Of 1761 analyzed patients, 49.3% experienced CRN at least once; 10% had persistent CRN.
  • Persistent CRN was associated with lower dual eligibility (aOR=0.45) and higher depression (aOR=1.55).
  • Younger age correlated with higher persistent and intermittent CRN; transient CRN was age-independent.

Conclusions:

  • Persistent CRN affects a substantial patient group, including those nearing end-of-life.
  • Further research is essential to understand CRN behavioral patterns in younger Medicare populations and inform interventions.
