The Cochlea

The cochlea is a coiled structure in the inner ear that contains hair cells—the sensory receptors of the auditory system. Sound waves are transmitted to the cochlea by small bones attached to the eardrum called the ossicles, which vibrate the oval window that leads to the inner ear. This causes fluid in the chambers of the cochlea to move, vibrating the basilar membrane.

Background:

  • Cochlear implantation (CI) can alter vestibular function.
  • The relationship between vestibular changes and CI performance is not fully understood.

Purpose of the Study:

  • To investigate if caloric vestibular responses post-cochlear implantation can predict hearing performance.

Main Methods:

  • Retrospective analysis of 43 patients undergoing CI with pre- and post-operative videonystagmography (VNG).
  • Caloric responses were compared pre- and post-implantation.
  • Prediction of AzBio +10 dB S/N scores was assessed.

Main Results:

  • A significant decrease in caloric response was observed in the implanted ear post-CI (p=0.02).
  • Pre- and post-implantation caloric responses predicted CI performance at various time points.
  • Changes in caloric response did not correlate with CI performance.

Conclusions:

  • Cochlear implantation impacts vestibular function, evidenced by altered caloric responses.
  • Vestibular function metrics before and after CI may predict aspects of CI performance variability.
