Background:

  • Fireworks are a significant cause of vision-threatening injuries.
  • The effectiveness of local legislation in mitigating firework-related ocular trauma remains unclear.

Purpose of the Study:

  • To evaluate the association between local firework legislation and the incidence of ocular trauma.
  • To compare the odds of firework-related eye injuries in areas with and without firework bans.

Main Methods:

  • A case-control study was conducted over 8 years (2016-2022) at a level 1 trauma center.
  • 230 patients with ocular trauma were analyzed, with firework ban status of residence as the primary exposure.
  • Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated to assess injury risk.

Main Results:

  • The odds of firework-related ocular trauma were 2.0 times higher for residents in areas where fireworks were legal compared to banned areas (P=.01).
  • Younger patients (<18 years) and male patients had significantly higher odds of firework injuries.
  • Firework-related injuries were more likely to be vision-threatening (57%) compared to non-firework injuries (40%).

Conclusions:

  • Local firework bans appear to be associated with a modest reduction in firework-related ocular trauma.
  • Despite the association, further research is needed to identify strategies for more substantial injury reduction.
  • Legislation and public awareness campaigns may play a role in preventing these preventable injuries.
