Background:

Purpose of the Study:

  • To describe the epidemiologic characteristics, clinical manifestations, and treatment of nail psoriasis in China.
  • To investigate the prevalence of fungal infections and arthritic complaints in nail PsO patients.
  • To provide data for improved global patient management.

Main Methods:

  • A prospective, multicentre observational study was conducted across 25 hospitals in China from August 2021 to August 2022.
  • Data on demographics, clinical signs, fungal detection, arthritic symptoms, and treatments were collected from 817 patients with nail PsO.
  • Statistical analysis was performed to identify correlations and significant associations.

Main Results:

  • Nail psoriasis affected 95.29% of fingernails and 57.18% of toenails, with pitting and subungual hyperkeratosis being most common.
  • Onychomycosis was present in 13.26% of patients, predominantly in toenails. Articular symptoms were reported by 12.17% of patients.
  • Only 2.30% of patients received treatment, with biologic therapy being the most common for cutaneous psoriasis with nail involvement.

Conclusions:

  • Nail psoriasis exhibits distinct features in fingernails versus toenails, with toenail involvement and co-existing onychomycosis requiring specific focus.
  • Articular symptoms in psoriatic patients are linked to specific nail changes, emphasizing the need for comprehensive assessment.
  • The low treatment rate underscores the necessity for developing and advocating for more effective nail psoriasis therapies.
