    Background:

    • This report details a rare case of intramucosal carcinoma.
    • The carcinoma arose within an otherwise typical solitary juvenile polyp.
    • The patient did not have the multiple juvenile polyposis syndrome.

    Findings:

    • Intramucosal carcinoma is a rare but documented complication of juvenile polyps.
    • Malignant transformation can occur in juvenile polyps independent of the genetic syndrome.
    • This finding expands the understanding of potential malignant pathways in juvenile polyps.

    Implications:

    • Solitary juvenile polyps should be considered a potential site for malignant change.
    • Increased surveillance may be warranted for individuals with juvenile polyps, even without the syndrome.
    • Further research is needed to elucidate the mechanisms of malignant transformation in these lesions.
