    Summary

    Spontaneous cervical internal carotid artery (ICA) dissection is rare but serious. Many patients experience poor recovery due to thrombosis, suggesting a severe prognosis for this stroke cause.

    Background:

    • Cervical artery dissection is a cause of stroke, particularly in younger individuals.
    • Spontaneous dissection of the internal carotid artery (ICA) can lead to cerebral infarction.
    • Clinical presentation of ICA dissection is often nonspecific, delaying diagnosis.

    Purpose of the Study:

    • To investigate the clinical course and outcomes of spontaneous cervical internal carotid artery (ICA) dissection.
    • To identify factors associated with recovery and recurrence after ICA dissection.
    • To evaluate the potential role of early heparin therapy.

    Main Methods:

    • Retrospective analysis of 1200 consecutive stroke patients.
    • Identification of 30 patients with spontaneous cervical ICA dissection.
    • Follow-up using Doppler ultrasonography and clinical assessment over a mean of 3.2 years.

    Main Results:

    • 2.5% of stroke patients had spontaneous cervical ICA dissection.
    • Only 17% presented with classic symptoms (headache, oculosympathetic paresis).
    • 12 survivors had good recovery with ICA reopening; 11 had poor recovery without reopening.
    • Large infarcts were linked to ICA thrombus and distal emboli.
    • Recurrence of dissection was rare (1 patient).

    Conclusions:

    • Spontaneous cervical ICA dissection often carries a severe prognosis, contrary to some opinions.
    • Intraluminal thrombosis and organization may impede ICA reopening and recovery.
    • Early heparin therapy's benefit is unproven but may prevent clotting.
    • Diagnosis often requires angiography due to nonspecific initial symptoms.
