Hearing and middle ear function in rheumatoid arthritis.

S Elwany, A el Garf, T Kamel

    The Journal of Rheumatology

    Summary

Rheumatoid arthritis commonly affects middle ear function, causing reduced compliance in most patients. Sensorineural hearing loss may occur, but its link to rheumatoid arthritis activity remains unclear.

    Area of Science:

    • Otolaryngology
    • Rheumatology
    • Audiology

    Background:

    • Rheumatoid arthritis (RA) is a systemic autoimmune disease.
    • Potential effects of RA on auditory function are not fully understood.

    Purpose of the Study:

    • To investigate hearing and middle ear function in patients with rheumatoid arthritis.
    • To identify potential correlations between auditory changes and disease characteristics.

    Main Methods:

    • Audiological assessments including tympanometry and pure-tone audiometry were performed.
    • 68 patients diagnosed with rheumatoid arthritis were included in the study.

    Main Results:

    • Reduced middle ear compliance was observed in 70.5% of patients, suggesting tympanossicular chain stiffening.
    • Conductive hearing loss was rare (1.40%).
    • Moderate high-frequency sensorineural hearing loss (20-30 dB) was present in 29.40% of patients, with some evidence of cochlear involvement.

    Conclusions:

    • Rheumatoid arthritis frequently impacts middle ear compliance.
    • Sensorineural hearing loss in RA patients may involve cochlear pathology.
    • The relationship between sensorineural hearing loss, RA disease activity, and treatment requires further investigation.
