  • A 60-year-old woman developed binocular vertical diplopia (double vision) after receiving HA filler injections in her right tear trough.
  • Clinical examination revealed right hypertropia, indicative of right inferior rectus paresis, likely caused by vascular injury to the infraorbital artery.

Findings:

  • The patient's double vision resolved completely following prompt administration of hyaluronidase and oral steroids.
  • This suggests that early intervention can effectively manage specific vascular complications associated with HA fillers.

Implications:

  • This case underscores the critical importance of recognizing and diagnosing potential ocular complications following HA filler injections.
  • Timely and appropriate management, including the use of hyaluronidase, is essential to mitigate adverse effects and preserve visual function.
