    Background:

    • Parkinson's disease is a progressive neurodegenerative disorder.
    • Levodopa is a primary treatment for Parkinson's disease symptoms.

    Purpose of the Study:

    • To evaluate the long-term impact of levodopa introduction on Parkinson's disease progression and outcomes.
    • To assess the incidence and progression of motor complications with levodopa therapy.

    Main Methods:

    • Comparative study of Parkinson's disease patients over two 15-year periods (pre- and post-levodopa introduction).
    • Analysis of disease duration, mortality, disability, and motor complications (abnormal involuntary movements and fluctuations).

    Main Results:

    • Levodopa treatment increased disease duration by 3-5 years and reduced death/disability 1.5-3 fold.
    • 24% experienced abnormal involuntary movements (AIMs) and 29% had severe fluctuations, which stabilized after 3 years.

    • Motor complications like AIMs and fluctuations are manageable and their severity plateaus, with age of onset being a key predictor.
