Background:

Medical specialty selection is complex, with neurosurgery often facing student hesitancy.
Perceived difficulty and demanding schedules deter some students from neurosurgery.
Understanding influences on neurosurgery career choice is crucial for medical education.

Purpose of the Study:

To identify factors affecting medical students' and interns' intentions toward neurosurgery.
To explore motivations and deterrents for pursuing a neurosurgery career.
To analyze these factors within two institutions in Saudi Arabia's eastern province.

Main Methods:

Cross-sectional study utilizing an anonymous, validated electronic questionnaire.
Data collected from medical students and interns in Saudi Arabia's eastern province.
Statistical analysis included descriptive statistics and chi-square tests.

Main Results:

34.1% of participants showed interest in neurosurgery; only 16.3% had comprehensive knowledge.
Motivations: high income, prestige, patient impact. Deterrents: stress, work-life balance, competition, neurophobia.
Interest decreased with age and academic progression, except for early medical students.

Conclusions:

Early exposure, clinical training, and personal factors significantly shape neurosurgery career preferences.
Addressing perceived difficulties and work-life balance concerns is vital to attract more students.
Enhancing neurosurgery's appeal requires considering personal and lifestyle factors alongside academic ones.
