Competency-based, time-variable graduate medical education (GME) offers flexible physician training. A pilot program demonstrated the feasibility of the "Promotion in Place" model, advancing or delaying graduation based on demonstrated competence.
Area of Science:
- Medical Education
- Graduate Medical Education
- Competency-Based Education
Background:
- U.S. residency programs are largely time-based, leading to varied assessments and limited individualization.
- Gaps in physician readiness for unsupervised practice are noted across specialties.
- Logistical and regulatory barriers hinder the adoption of competency-based, time-variable (CBTV) graduate medical education (GME).
Purpose of the Study:
- To describe a vision and implementation model for CBTV-GME.
- To introduce the
Main Methods:
- The
Main Results:
- One program successfully piloted the PIP model, demonstrating feasibility.
- Implementation tools and templates were developed.
Conclusions:
- Larger-scale implementation and longer-term assessment are necessary.
- The impact and generalizability of the CBTV-GME model require further evaluation.
