Introduction
An electrocardiogram (ECG) is a diagnostic tool for identifying cardiac conditions such as arrhythmias, conduction abnormalities, and myocardial ischemia.
Definition

Summary

Extracting VDD (atrial and ventricular sensing/pacing) leads may increase transvenous lead extraction complexity due to patient factors, but does not raise complication risks. Experienced operators can achieve favorable outcomes with specialized techniques.

Background:

  • Ventricular-Dual-Chamber-Defibrillator (VDD) leads are infrequently implanted, leading to limited operator experience in their extraction.
  • The complexity and complication rates associated with transvenous lead extraction (TLE) involving VDD leads remain under-investigated.

Purpose of the Study:

  • To evaluate if VDD lead removal during TLE procedures increases procedural complexity or the risk of major complications.
  • To identify factors contributing to the complexity of VDD lead extraction.

Main Methods:

  • A retrospective analysis of 3808 TLE procedures was conducted.
  • Data from 103 patients undergoing VDD lead extraction were compared with those without VDD leads.

Main Results:

  • TLE involving VDD lead removal was associated with longer procedure times, a higher rate of complicated extractions, and increased need for advanced tools.
  • Factors contributing to complexity include longer implant duration, higher prevalence of abandoned leads, and younger patient age at implantation for VDD systems.
  • Crucially, VDD lead extraction did not significantly increase the risk of major complications (1.94% vs. 2.34%).

Conclusions:

  • VDD lead extraction can be considered a risk factor for increased procedural complexity, not major complications.
  • This complexity is attributed to patient-specific characteristics rather than the VDD lead itself.
  • Operator expertise, team experience, and specialized techniques are vital for successful VDD lead extraction, even with older models.
