This case study highlights a rare retroperitoneal solid pseudopapillary neoplasm (SPN) with multiple liver metastases in a young patient. Surgical resection proved effective, emphasizing its role in managing SPN metastases.

Area of Science:

  • Oncology
  • Surgical Pathology

Background:

  • Solid pseudopapillary neoplasm (SPN) is a rare, low-grade malignant tumor of the exocrine pancreas.
  • While SPN can metastasize, multiple extrapancreatic metastases are exceptionally uncommon.

Observation:

  • A 13-year-old male presented with abdominal trauma, revealing a retroperitoneal SPN with three hepatic metastases on enhanced CT.
  • Elevated serum tumor markers (CA125) and circulating tumor cells were noted.
  • Splenomegaly was also observed.

Findings:

  • The patient underwent successful resection of the retroperitoneal tumor, spleen, and liver metastases.
  • Postoperative confirmation of multiple liver metastases from retroperitoneal SPN.
  • One-year follow-up showed no tumor recurrence.

Implications:

  • This case underscores the importance of thorough medical imaging and serum tumor marker evaluation in pediatric patients with suspected SPN.
  • Surgical intervention is presented as a viable and potentially curative treatment for multiple SPN metastases.
