This case study highlights a rare retroperitoneal solid pseudopapillary neoplasm (SPN) with multiple liver metastases in a young patient. Surgical resection proved effective, emphasizing its role in managing SPN metastases.

Background:

Solid pseudopapillary neoplasm (SPN) is a rare, low-grade malignant tumor of the exocrine pancreas.
While SPN can metastasize, multiple extrapancreatic metastases are exceptionally uncommon.

Observation:

A 13-year-old male presented with abdominal trauma, revealing a retroperitoneal SPN with three hepatic metastases on enhanced CT.
Elevated serum tumor markers (CA125) and circulating tumor cells were noted.
Splenomegaly was also observed.

Findings:

The patient underwent successful resection of the retroperitoneal tumor, spleen, and liver metastases.
Postoperative confirmation of multiple liver metastases from retroperitoneal SPN.
One-year follow-up showed no tumor recurrence.
