Summary

The harmonic scalpel (HS) significantly reduces operative time and blood loss in neck dissections for head and neck cancers compared to conventional methods. However, it shows no advantage in hospital stay or complications.

Background:

  • The choice between harmonic scalpel (HS) and conventional hemostasis for head and neck cancer treatment is based on clinical outcomes.
  • Neck dissection is a common procedure in managing head and neck cancers.

Purpose of the Study:

  • To evaluate and compare surgical outcomes of neck dissection using HS versus conventional hemostasis.
  • To conduct an updated meta-analysis of the available literature on these techniques.

Main Methods:

  • Systematic review and meta-analysis of randomized control trials (RCTs) published up to December 31, 2021.
  • Searched PubMed, Scopus, and Cochrane Library following PRISMA guidelines.
  • Analyzed operative time, intraoperative blood loss, hospital stay, drain parameters, and postoperative complications.

Main Results:

  • Included 10 RCTs with 558 patients.
  • HS group showed significantly shorter operative time (MD = -23.21 min, P < .0001).
  • HS group demonstrated significantly lesser intraoperative blood loss (MD = -61.53 ml, P < .00001).

Conclusions:

  • Harmonic scalpel use in neck dissection reduces operative time and intraoperative blood loss compared to conventional hemostasis.
  • No significant difference was found in length of hospital stay, drain stay, or postoperative complications.
  • Further high-level RCTs are needed to fully elucidate the effectiveness of HS in head and neck cancer treatment.
