Background:

  • Bloodstream infections caused by carbapenem-resistant bacteria are a growing global health concern.
  • Solid-organ transplant recipients face a higher risk of these severe infections.

Purpose of the Study:

  • To compare clinical outcomes of carbapenem-susceptible and carbapenem-resistant Enterobacteriaceae bloodstream infections.
  • To identify risk factors for carbapenem resistance in solid-organ transplant recipients.

Main Methods:

  • Retrospective descriptive study of solid-organ transplant recipients (age ≥18) from 2015-2022.
  • Analysis of Enterobacteriaceae-positive blood cultures from medical records.
  • Identification of risk factors using logistic regression.

Main Results:

  • Carbapenem resistance rates increased from 15.8% before 2020 to 39.5% after 2020 (P = .007).
  • Significant risk factors included high Pitt bacteremia score, Charlson comorbidity index, chronic liver disease, septic shock, hypotension, delayed bacteremia onset, recent hospitalization, and prior invasive procedures.
  • Independent risk factors for carbapenem resistance were bacteremia ≥48 hours post-hospitalization and hospitalization within the past 3 months.

Conclusions:

  • A significant rise in carbapenem resistance among Enterobacteriaceae bloodstream infections in transplant recipients was observed.
  • Bacteremia developing ≥48 hours post-hospitalization and recent prior hospitalization are critical risk factors.
  • Emphasizing infection control, minimizing hospitalization duration, and adherence to prevention methods are crucial for managing these nosocomial infections.
