Background:

True brachial artery aneurysm is a rare condition with potential for severe neurological and vascular complications.
Identifying demographic characteristics, etiologies, and clinical manifestations is crucial for effective management.

Approach:

A systematic review of literature was conducted adhering to PRISMA guidelines.
Databases searched included MEDLINE, EMBASE, and Cochrane for studies published up to May 31, 2022.
Data from 113 articles involving 157 patients were analyzed descriptively.

Key Points:

The mean age of affected patients was 43.1 years, with a male predominance (73%).
Common presentations included localized swelling, pain, distal ischemia, and median nerve compression.
Renal failure patients with a history of arteriovenous fistula creation and immunosuppression were most commonly affected (51.5%).
Other causes included idiopathic (17.1%), trauma (8.2%), connective tissue disorders (5%), and vasculitis (4.5%).
Aneurysmectomy with reversed great saphenous vein graft was the preferred treatment (50.3%).

Conclusions:

True brachial artery aneurysms, though rare, pose significant risks and are strongly associated with arteriovenous fistulas and immunosuppression in renal failure patients.
Open surgical repair, particularly aneurysmectomy with vein grafting, remains the primary treatment modality.
Further research comparing open versus endovascular interventions is needed to refine future treatment strategies.
