  • To evaluate the current use of COS in the development of clinical practice guidelines.

  • Searched for clinical guidelines relevant to ten high-quality COS across multiple countries.
  • Matched guideline scope (condition, population, outcome) with COS.
  • Calculated the proportion of guidelines referencing COS and the concordance of COS domains with guideline PICO statements.

Main Results:

  • 38 guidelines with 170 PICO statements were analyzed.
  • No guidelines explicitly mentioned or referenced relevant COS.
  • The median proportion of core outcomes covered by guidelines was 30%.

Conclusions:

  • COS are not routinely informing clinical guideline development.
  • Limited concordance exists between outcomes in guidelines and those in COS.
  • Further research is needed to identify barriers and facilitators for COS implementation in guideline development.
