Lichen planopilaris (LPP), a form of follicular lichen planus, can coexist with female pattern hair loss, mimicking alopecia areata. Early diagnosis and treatment are crucial for managing this complex hair loss condition.

- Dermoscopy-guided biopsies from androgen-sensitive and insensitive areas aided diagnosis.
- Combined LPP and female pattern hair loss can present as diffuse alopecia.
Implications:
- Prompt treatment initiation is vital to halt disease progression in LPP.
- Long-term follow-up is necessary as clinical resolution may not indicate disease absence.
