Background:

Pyoderma gangrenosum (PG) is a neutrophilic inflammatory condition often associated with autoimmune diseases and malignancies.
Previous studies have linked PG to various cancers, including breast cancer subtypes like ductal carcinoma.
The association between PG and cutaneous metastatic lobular carcinoma remains under-documented.

Observation:

An 84-year-old woman presented with metastatic lobular carcinoma on her chest wall and pyoderma gangrenosum on her lower extremities.
Initial lower extremity ulcerations were misdiagnosed, worsening despite treatment.
Skin biopsies confirmed metastatic lobular carcinoma and pyoderma gangrenosum.

Findings:

This case presents a rare concurrent manifestation of pyoderma gangrenosum and cutaneous metastatic lobular carcinoma.
The patient experienced worsening PG symptoms in conjunction with metastatic breast cancer progression.
Metastatic malignancy was identified as a potential trigger for the pyoderma gangrenosum flares.

Implications:

This case underscores the importance of considering metastatic malignancy in patients with atypical or worsening pyoderma gangrenosum.
It highlights a potential, albeit rare, link between lobular breast carcinoma metastasis and pyoderma gangrenosum.
Further research is warranted to explore the relationship between specific cancer types and PG development.
