Adolescent endometriosis differs in patients with obstructive Müllerian anomalies. These patients present earlier and have a higher rate of hydrosalpinx (fluid in the fallopian tube).
Area of Science:
- Reproductive Medicine
- Gynecology
- Pediatric Surgery
Background:
- Endometriosis in adolescents with obstructive Müllerian anomalies is not well understood.
- Obstructive Müllerian anomalies can impact reproductive health and pelvic anatomy.
Purpose of the Study:
- To compare the clinical characteristics of endometriosis in adolescent patients with and without obstructive Müllerian anomalies.
- To identify differences in presentation, associated conditions, and recurrence rates.
Main Methods:
- Retrospective analysis of 43 adolescent patients (<24 years) undergoing surgery.
- Group 1: 12 patients with obstructive Müllerian anomalies and endometriosis.
- Group 2: 31 patients with endometriosis without obstructive Müllerian anomalies.
Main Results:
- Endometriosis occurred in 50% of obstructive Müllerian anomaly cases (Herlyn-Werner-Wunderlich syndrome, non-communicating uterine horn, cervical aplasia).
- Patients with obstructive Müllerian anomalies were significantly younger at surgery (17.8 vs. 23.1 years).
- Hydrosalpinx was significantly more common (66.7% vs. 0%) in the obstructive Müllerian anomaly group.
Conclusions:
- Adolescent endometriosis associated with obstructive Müllerian anomalies presents earlier.
- Obstructive Müllerian anomalies are linked to a higher incidence of endometriosis and hydrosalpinx in adolescents.
- Early diagnosis and management are crucial for this patient population.
